Belial spitting out a guaranteed Ancestral Unique sounds amazing until you realize the grind behind it is half the fight. If you're chasing the Diablo 4 Secret Cow Level and Boss Trophy loop in Season 13, don't sleep on sorting stash space, loot filters, and even D4 items cheap options before you start burning Torment runs. The short version: Boss Trophies are for targeted Unique fishing, while the cow quest is a weird multi-zone puzzle with some very dumb failure points.
I started the cow hunt thinking it was just another meme chain. Nope. First, the game asks you to kill 666 cows across Sanctuary, then you're hunting three white-quality relics from Scosglen, Dry Steppes, and Hawezar. After you clean them at the Ked Bardu fountain and turn them in at the Oxen Statues, you get the Strange Key for the Forlorn Hovel and grab the Stamina Potion. Don't run this with a strict loot filter, because those relics can vanish from view like trash rares after a Helltide.
Boss Trophies only drop in Torment, and they come from Lair Bosses, World Events, Tree of Whispers jobs, and Mercenary Bartering. Once you finish the Lord of Hatred campaign, the Horadric Cube opens at Temis in Skovos. Five matching trophies in Amalgamation spits out a random Unique from that boss pool, with a shot at Greater Affixes. I burned through Abhorrent Hearts on my Spiritborn test setup, and yeah, Sepazontec and Rod of Kepeleke are exactly why that farm still has legs.
Target farming is still cleaner when your build needs one exact piece. Belial needs two Betrayer's Husks, but the payoff is nasty: beat him and you can pick another boss table for an Ancestral Unique with at least one Greater Affix. Mephisto asks for the Crux of the False Prophet and is still the Mythic Seal source for Talismans. Lord Zir can hand you X'Fal's Corroded Signet or The Umbracrux, while Grigoire is where I'd go for Paingorger's Gauntlets or The Mortacrux.
Nahantu is where the quest gets fussy. You need the Jabbering Gemstone from grave emotes, the Rusted Old Bell from visiting 12 statues on foot, and the Crooked Staff from killing Super Elite Blood Lightning with only basic Attack, no gear, and no skill points. No shot, that last bit is the worst part. Warlocks and Paladins can still get bricked by class item/passive damage weirdness, and after the recent Blizzard bug fixes around Skovos training dummy behavior, I'd still take that workaround talk with a grain of salt.
Skovos then pushes you into the Outcast's Journal chain and the Cedar Tablets in Kyovashad. Set Fear for the Hierophant's Skull and follow the Large Crow in the Ruins of Broken Reason. Set Anger, farm the Amazon armor set from Corrupted Amazons, and you're after the Mother's Rose. Set Pride, kill N'facl the Jailer of Angels with an Amazonian Bolt, then use the Hadopelagic Key while Greed is active to fish up the coffin in the Temple of the Deathspeaker for Neyrelle's Hand.
Honestly, yes, but only if you like oddball grinds. Cube the Stamina Potion, Rusted Bardiche, and Neyrelle's Hand to get the Trophy of the Faithful Cache, Moo Emote, and There is No Cow Level achievement. The main Isle of Scylara portal only pops on Tuesdays, but using Moo near other players can open the daily Skovos version. Inside, the Cow King drops Cow King's Crown and Prime Ribs, though nobody has pinned down what Prime Ribs do yet; if you'd rather save time gearing alts, U4GM is the kind of marketplace players use for game currency and item services. Your mileage may vary, but for Greater Affix chasing and pure Diablo nonsense, this is the most Season 13 grind we've got.
